The Australian Transport Safety Bureau (ATSB) preliminary investigation report into the depressurisation incident and diversion to Manila of a Qantas B747-400 on 25 July, indicates that one of the aircraft’s 13 passenger oxygen system cylinders failed, causing damage to the fuselage.
